Forms for urgent care and medical cannabis are below for you to fill out, ALL other booked appointments will be emailed their required forms to complete after booking the appointment.  

Please make sure to check the "Forms" section below the "Appointment" section on this page and complete your forms before your appointment (CANNABIS and URGENT CARE). Failure to do so will result in the cancellation of your appointment, and you will be required to rebook.

More than one missed or canceled appointment due to not following instructions will result in the need to prepay to book future appointments.

We are very busy, and if you are unable to complete the forms or make your appointment, please notify the office as soon as possible. Failure to do so means someone else may miss the opportunity to book that time slot, so please be considerate of others. Book appointments online.
